run time error 1004: application defined or object defined error

This is supposed to start a log by having the user input their name and then the marco puts their name and a time stamp on a worksheet. I have a similar code that does this on workbook close but I am running into a run time error when I am trying to use the code on workbook open. Any help would be appreciated

The code breaks if i include the lines that input the name collected from the input box and the time stamp, but each work correctly separately
