need help inputting data from a form

I am having trouble trying to work out how to extract data from a data form and putting it in the correct place, I dont know if im even approaching it the correct way. The data form is in the 'Enter Match Data' sheet and I dont know how to copy the data entered in that form into the 'database' sheet to the correct member under the correct column heading. Also the data from the form must be added to any data that is already in the database, the data from the form is all numerical so the values in the database should keep getting bigger each time the data form is filled out for a specific person. Thanks for any help
