How to automatically assign a macro to a custom button?

    Hi there,

I'm currently trying to create a macro that runs as the work book opens, which will create a custom menu button and assign a macro to it. I can't seem to find out how to assign a macro to the button automatically with VBA code. The code I have to make the button appear is

So I just need maybe one or two lines that will call a macro and make my userform appear when the button is clicked. I also need for the macro to delete as the workbook closes. Recording the manual process yielded no code.
