Button to create a new workbook with name as Column B

Dear Friends,
My Actual Requirement is when I manually enter data in 1 excell sheet and click on create button.
it must create a new workbook from a template where my template name is Revanth.xlsm (Sorry dont remember the extension of macro enabled template) it contains 9 sheets and each sheet is formatted individually. so the main thing it has to do is create new workbook from template and save it in folder D:\ YRK And copy entire row to First sheet of the created work book into second row. and create a hyper link to that work book for column B and generate a new Create button For next row. so for each row there will be a create button at the end
Please find the Attachment for a clear Idea. I an unable to attach the template since it is un supported.
