Okay, I'm the stubborn type and I've searched and searched for answers with no luck. I'm only a little familiar with VBA and I need some advice if anyone has the time. I have a workbook that is about collecting survey data. There is a sheet (the first) that users will enter data into a few form field text boxes, and then click a button to add that info to one of three other sheets in the same workbook, but based upon the choice of two of the drop-down boxes. I'm attaching a de-identified version for review (just say 'no' to HIPPA). My intent, as I started to say already, is for the user to simply enter a few numbers and a little text, then have all of that info transferred to the appropriate cell in another sheet (in a new row) based upon the type of survey (this is the drop-down that has either "type 1, type 2, or type 3") AND based upon another drop-down that is about the time-frame (drop-down that has "T1, T2, T3, etc.") as an option. So, if 'type 1' and 'T2' (time 2) are selected then the info would go to the worksheet titled 'Type 1' and in the area of the table for 'T2'. And, of course, I'm trying to attach this to a command button. Please let me know what other information would help this become more clear. Thank you for your time.
