Help with lookup and to populate a form

Hello All,
I am looking to add a command button into my form that will look up data in a field and populate the rest of the text boxes in the form. I need help with the VB commands.
Any help would be greatly appreciated.

Name, Department, Budilding, Floor .etc.

I would like to enter a name and have the rest of the fields in the form be populated

I was able to figure it out....now I am just attempting to look up data via 2 fields such as Lastname and date of birth
