Hello All,
I am looking to add a command button into my form that will look up data in a field and populate the rest of the text boxes in the form. I need help with the VB commands.

So in the worksheet "Survey", which column would be your lookup?
Name, Department, Budilding, Floor .etc.

I was able to figure it out....now I am just attempting to look up data via 2 fields such as Lastname and date of birth
