vba user form to create message depend on option button/check box

hello,
i've got some excel where i tupe two texts in two cells, then i choose from the list one of the options and after this in 4th cell i've got text ready to copy. Results depend on value choosen from list. Its quite easy i used data validation, vlookup and"&".
I want to create this by using user form and option button or check boxes instead of list. But its to complicate for me. Everything you can find in att file, also there is pic how i see it.
If you got any question feel free to write. I will be grateful for any suggestion or idea how to do this. Thanks in advance.

i tried to follow some code and it didnt work , for some reason the data on the form doesnt go into the sheet why is that?
the information in the form is for example range(a1).value = textbox1.value and yet nothing in a 1 why???http://www.excelforum.com/images/smilies/confused.gif

can someone please provide the whole coding to add new data into the sheet thanks! im using excel 2010
