How to add a template sheet for an index table

Hello, much appreciation for this forum and the people helping here, I wanted to ask for a solution for something important before I goes and works all hard and go crazy.
I've this template file for a lot of my project, I'm asking for the Pro's way of adding a sheet in my excel file - which have uniqe style, I've added the explanation in the best way I know - as an example in the file I've uploaded.
