userform for data entry into "Database"

Hi, here i have attached my sample sheet. i know my requirements are big. if u can solve at least one in that list pls tell me. thanks in advance.

my requirements.

1 userform for data entry into "Database"

2 chart/graph to analyze customer segment from "Existing Customer" and "Blacklist Customer" that will update itself automatically

3 identify duplicates when entering data in "Database", then prompt

4 if status "existing", then copy the line of info into another spreadsheet ("Existing") for ease of client management

5 if status "blacklist", then extract the line of info into another spreadsheet ("Blacklist Customer")

6 userform (with drop down menu for the GivenName and Surname/number) to update information of customer
