I'm sorry but I don't think it's possible.
You can open various Excel files on an iPad but a lot of the features are disabled.
I've not seen anything specific for Forms controls, but then again this is the first time I've seen anybody ask about them.
Perhaps if you checked some Apple forums somebody might be able to suggest some sort of alternative.

I think Norie is right. VBA shares an intermediate language with a bunch of other programming languages. However, that IL is converted into a machine language that Apple products won't be able to read.
