I have created the attached user form (exactly as the actual form should look) but I can't figure out how to get the user to use it and then print out a copy. Any help?

Do you mean you want to replicate the form on a worksheet?
Then populate the ' form' on the worksheet with the data from the userform and print it.

That's right, Norie. I didn't realize that the user won't be able to print the userform.

I can show you how to Print the UserForm...I can't tell you how to get the Users to use it. The UserForm is too intimidating.
Click the Option to print it out.

No, the printout of the userform won't work. It has to be an "actual form" (that will be faxed). Nevertheless, I appreciate your assistance.

I don't know what this meansThe Code prints out your "actual form" which then can be faxed.It has to be an "actual form" (that will be faxed).

Did you actually get the design for the userform from a paper form?
I've seen paper forms like this, with things like single boxes for each digit of a date/telephone no.
That doesn't really work with userforms, try entering a date or take a look at the attached image and you'll see what I mean.
