Prepopulating a UserForm

If this has been answered do please direct on finding the answer.
Have a UserForm that allows user to enter say 10 pieces of information on a product.
The last question on the UserForm is a Yes/No box - Does this product have more chemicals?
If the person clicks yes, I want to prepopulate a second UserForm with all but 3 pieces of information from the previous form that was filled out.
In other words this second UserForm will be prepopulated with duplicate information - like name of company, address, date, etc., however the 3 pieces of
information that need to be filled in will be different from what was filled out before. See?

Thanks for any suggestions or directions. Am able to do UserForms with no problem - but having difficulity with prepopulation.

so you would launch the Form2 if in Form1 they click on yes....copy this for each field you want to pre-populate.....
