Creating a Time Card Tracker for Team

Hello! I am attempting to create a time card tracker within Google Sheets. I am having team reps fill out a Google form that provides me with their name, a timestamp for when the form was filled out, and the punch type that they are doing (Clocking in, clocking out for lunch, clocking in from lunch, and clocking out for the day). I am then transposing the data into a sheet that organizes the daily punches per employee.

The issue that I am running into is getting the correct time stamp for the appropriate punch type with the associated date. Essentially I am trying to pull the timestamp from a cell if two criteria are met. My logical equation is:
Get timestamp if specified date is correct AND specified punch type is correct.

Sheets:
Form Responses - Raw data imported from form
Sheet 1 - Data sorted to team members
Organized List - Running report that will provide me with the desired break down for the team on a day by day basis.

Hello Erroneous Man and Welcome to Excel Forum.
I feel that the data on the Form Response sheet is better suited to analyzing than that on Sheet1.
With that in mind I suggest pasting the following formula into cell B3 on the Organized List sheet:

Once pasted drag the fill handle over to cell E3 and then, while B3:E3 are still selected, drag the fill handle down to E4.
While B3:E4 are still select press the Ctrl and c keys
Select cell G3 and press the Ctrl and v keys.
To see any results change the dates in B1 and G1 to 1/12/2021 and 1/13/2021 respectively.
