Data collection for a Community project

Hi, We provide meals within our community on a daily basis. For that purpose, we need to have certain information regarding the family members.

In the attached sheet, the HOF ID states the ID of our primary contact, and each family member is tagged with a unique ID, but their HOF ID remains the same. We want to be able to query the data with 1 HOF ID with multiple unique IDs and display it in a row view & not in a column.

Secondly, We want to create a Google form wherein we fill the HOF ID & unique IDs and this data is grouped in a column form based the HOF ID (i.e. any new entry with same HOF ID should create a new row before the start of another HOF ID). I could find the option to automatically group rows by selecting them & right clicking on group. But I would want to know if that is something that can be done with just entering the data in Google Forms.

I'm attaching a sample file of how we want the data to be displayed. But we need formulas that work with Google Sheets.

    Any help is greatly appreciated.

    Thanks

This is a google sheet question, you should be sharing a google sheet, not an excel workbook.

The excel file just has the basic data for reference, I'm looking for the solution in google sheets. Let me know if something about the data is not clear.
