    inserting excel cells into certain fields on a email

    I currently have a excel sheets that contains 450 usernames, passwords, and email addresses. I would like a simple way to make an email form and when I type in an email address in the to field have it look at the excel sheet and automatically insert the person's username and password into a certain area of the email message. Is that possible?
