Percent of 2 columns in pivot table..Need Urgent Help

I would like to do a pivot on the below table which will show me data as percent of the 2 columns "> 3 Score" and ">4 Score"

My table is :

My desired table should look line

> 3 Score > 4 Score
Aug-17 100% 97.5%

Attach a sample workbook (not a picture!). Make sure there is just enough data to demonstrate your need. Include a BEFORE sheet and an AFTER sheet in the workbook if needed to show the process you're trying to complete or automate. Make sure your desired results are shown, mock them up manually if necessary.

Remember to desensitize the data.

Click on GO ADVANCED and then scroll down to Manage Attachments to open the upload window.

In Pivot table use average instead of Sum or count and format cells as %
