Combined Scatter Plot?

HELP! I have attached data for which I would like to create a chart showing where each option (listed in file) lies in a range of severity in relation to the cost. There is a picture in the file for what I want the chart to look like.

Select the range C4:F6

insert a chart and change to xy-scatter
switch row/column
repeat switch row/column

Rest should be formatting of scales/gridlines/markers
