vlookup/if formula not working as expected

what I am trying to accomplish is to automatically fill in a work instruction based on a product picked from a data validation dropdown and a width criteria.

    If name = coated paper1 and the width = 3/4in, then cell $c$18 = "coated paper1" otherwise "n/a".

I know, its strange, but this portion of the work instruction is for the operator to label the product. Some widths are sold as is and others are used internally to make other products-yeah its convoluted.
