Mail Merge from excel file for labels not populating labels

Hello all,

Foremost, thank you for considering my post. All I want to do is simply take the information that is in columns a b and c, and have all this information appear on a single avery 5167 label. So for example, it should look like so..

Coastal AgroBusiness
Truck 611
252-414-3895

And then so on for all the other rows of data. I tried using the mail merge wizard, but it would not populate the data. Please help! Also, please explain your steps as how you solved this problem, because I want to be able to replicate.
