Using a Userform on a Word Document

I have a 3 page document that the only changes required are Client Name, Address Line 1, Address Line 2, Appointment Date, and Contact Number. I have setup a template so that we can go thru and make the changes but would love a userform that when you opened the document it asked you the questions and populated the fields throughout.

    All advice appreciated!

I take it the simple entry forms in Word are not suitable?

Sorry I should have went one step further in my request. I am currently using a template. The fields of information are duplicated and scattered throughout the document. IE the Client Name is in there 4 times, the address is in there twice, and the appointment date is in there twice. I was hoping to answer what the fields are and it automatically populate accordingly.

You can set form fields to take the value of another using REF fields, something like {REF Name}; that field will update to whatever value is entered into the 'Name' field so no need to have to enter it multiple times.

Just laying out some alternatives but if you really want a Userform then please upload a sample of your template and I'll knock up a basic form for you.

If you have your userform populate custom document variables or custom document properties with the data, you can use DOCVARIABLE fields or DOCPROPERTY fields, as appropriate, to reference them wherever you want. Then it's just a matter of using ActiveDocument.Fields.Update to refresh the data. The essential difference between document variables & properties is that the latter can be edited through the GUI.

Formfields only work if the document has forms protection applied. All you're using them for in this scenario is as bookmark containers, in which case one might as well write direct to bookmarks, then update the cross-references, without the bother of the formfield restrictions.
