    A survey with activex option button

    Hi

    I'm tryn to make a survey for my company so other companys could fill them out.
    The word document I'm working in has 8 Questions and each question contains 4-5 option buttons.
    My problem is that I can only have 1 option button ticked on 2 pages, but what I want is that I can have 1 option button ticked for each answer so that would be 8 option buttons clicked.
    When I click 1 the other one is unticked.

    How can I make it so you can tick an option button under every question?
    And also the fileds at the end of the survey where you insert your name, company and date.
    Is there an easier way to do this?

    Also the question 9 I made a table box where you can write comments in...basicly anything is that ok? Or should I use some other option for that?
    Or is there any other easier way to make a survey

    Please help.

    For such a survey, you should probably be using dropdown formfields for each item, so that the users can only select one item from each group. It would also make the form smaller. As using formfields requires forms protection to be applied, you'd probably also need to use a Section break before the table and leave that Section unprotected. Finally, I note that your document is saved in the docx format and contains some content controls on the last page. People using Word 2003 and earlier may not be able to open docx files and, even if they can, they will not be able to use the content controls.
