Hi,
I have got the attached records. The first worksheet is showing the raw data, and its not in correct pivot table format.
Column A "Date completed" is the unique identifier. Need to create pivot table to show the response choice (column C) in corrospondense to the question (column B).
The 2nd worksheet shows how it is supposed to look like when completed properly.

I can't see how you could do this in a PivotTable. However, here's a workaround including a PivotTable with only row and column headings. Results are to the right of the PivotTable.
Be aware that the formulas are array forulas (ctrl + shift + Enter).
Hide columns F-M in my attachment and the result is similar to what you ask for.
