Adding blank column to Pivot Table

I have a pivot table with two rows (Physician, Surgeons), 3 columns (CM, CYR, PYR). I want to insert two columns before the CM columns with a blank string. I need to insert a column before the CYR and PYR columns, also blank.

Can this be done?

You could add three calculated items to your CM/CYR/PYR field in your pivot table, with 0 as the formula, and then hide zero values in the pivot table (see Option 1 in attached example)

Or you could add three "blank" values for each Doctor type value (Physicians / Surgeons) in your source data (see Option 2 in attached example)
