How do I get data for the reason codes broken down by reason and time in a pivot table

I'm not sure exactly what you are after but I gave it a shot...

what I did was fill in your data so you can pivot it. I'm sure if this a solution you can work with. I was then able to pivot by date (Rows), Reason (Columns) and count of reason (again not sure what you are looking for in the PT)...
