3 Data Series Line Graph Help

I need help in plotting my 3 data series.
Goal is a constant number
My score changes
Dept AVG changes.

I think it would help if we understood exactly what difficulty you are having. Are you able to insert a chart? If, so what is right and wrong with the chart Excel creates? Is it related to formatting one or more chart elements? How I would expect to create a line chart from this:

1) Delete/Clear A2. For some reason, Excel likes to see the upper left corner of a chart data source empty when a chart like this is created.
2) Select A2:D14
3) Insert -> Line chart -> Line chart
4) Format and edit chart as needed.
5) Enter "Month" into A2, if I want that value present in the final spreadsheet.
