Attached is my interpretation of what you want. If this doesn't represent the solution you are seeking, then I suggest uploading a spreadsheet with some sample data that is representative of the actual data and then manually mock up the output you would like the pivot table to display.
To upload a spreadsheet (not a picture or pasted copy) click on the GO ADVANCED button below the Quick Reply window and then scroll down to Manage Attachments to open the upload window.
