Pivot not displaying/pulling exact numbers

Attached is my spreadsheet I am trying to work with. I am sure there is a very simple way to do this, but I am stumped. In the pivot table, I want to have the Facility Name on the left and the actual short term score it received (See Database I:I) on the right. The problem is because some facilities are named the same. What can I do to remedy this and still make it look clean and readable? Thanks!
