Line Chart auto update with new entry

attached is my sample spreadsheet to help clarify this. (i hope)

I'm looking to create a line chart that will automatically update as new 'entrys' are posted. but i'm also having trouble getting the right data in the right spot.

the x axis will be the DATA COMPLETE. the y axis will be COMPARISON. there are 5 possible TEAMS that will be entered in weekly. I want the chart to show a line for each team and to add a point on the chart when the data is entered.

Can a chart sort and organize by a value? in the attached example, in rows 22 thru 37, there are varies entrys for all different teams. But i need the chart to combine all TEAM A's, B's, C's etc.
