    Match Multiple Values with Multiple Values from Another Table - Return a Single Value

    Good Afternoon All,
    I am trying to look & match key values from 2 areas of one table with two areas of another table; in turn, it'd return one value based on the lookup table...

    The attached worksheet can shed some light on what I'm trying to accomplish.

    Is there a formula that'd work for this?

    Any info would be IMMENSELY appreciated!

    Their profile says they're using Excel 2003 but the sample file they posted is in the *.xlsx format.
