Look up row header based on column and table answer

Hi,

I need help passing in a customer (row 1, column headers) and a number of points (data in B2:F14) and getting back the row header value in Column A. For example, if I pass in customer 2 and points 9, the result would be 72.5%. For example, if I pass in customer 4 and points 12, the result would be 67.5%. In the scenario in which the same customer has more than one of the same points entry (e.g., the first three rows of customer 5 with 10), I would want the highest number in column A. Solutions greatly appreciated!
