Like so.... (see attached).
I removed the blank rows at the top to make the formulas a little easier to read.
This only works for UNIQUE values in the table. Duplicates would not be discovered.

Many thanks for your responses. JBeaucaire, this is a simple form i made in attachment file, actually i am working with
many value and some value have duplicate name in table so, i don't think to replace any name in cells to convert to unique
value as you said.
I think answere from Jaslake suitable for what i'm find for. I have tested in complex excel form like mine and
this vba code is working like what i mean. Thank's Jaslake for your solution.
