    simple vlookup, index&match

so i wanted to figure out how to do this so i could be assured of its accuracy and reliability and i figured a vlookup or index & match formula would best do that....

i've attached the file, please refer to the 'report list' tab, and note the 3 comments i made...
essentially i'm just populating the list there with all my reports from the 'report tracker' tab, however many there may be (that will vary month to month, so the number of rows in the 'report tracker' tab isn't a constant..it will change..and so the formulas created in the 'report list' tab will have to factor in that variable..

    any help is always appreciated..thanks
