Problems with COUNTIFS

Is there anyone with the relevant know how on how to solve my irritating problem please?
i have ran a trial on a certain equation which works, however when i try to use it on a larger scale within a database i am creating at work, i receive the #VALUE error. I am not too sure as to what is causing this error, maybe it could be because the information is spread across a few sheets, below i'll give you some details on what i am after.

Criteria:
I want to be able to make charts based on two pieces of information (Date and Error Code) for each month so that i can find some correlation between the two. I ran a trail of the code that i wanted to use on a separate spreadsheet, which worked a treat, however now i have put it into my database its not interested. ill provide some screens below.

Any help shed would make me immensely grateful, I have been racking my brain for a while now on how to amend this, thanks in advance. Im sorry if i have missed anything, thanks again.
