How can I simplify my formulas?

I have added some formulas that use multiple countifs in the same cell. Is there a way to simplify them by possibly using a different formula or a combination of formulas? I have attached the workbook and not quite sure how to explain what it does other than someone just looking at my workbook. I have to log welds made on piping. I have a sheet that calculates the total welds by weld type. It also calculates total percentage of welds x-rayed and total rejected x-rays and gives a percentage for each. I have on the same sheet another table that does the same thing but filters by date. The tables update each time new information is added. The way I have used the countifs seems to be a messy way of doing things and hope someone is able to give me a better way of doing what I'm trying to do.

I'm not too sure if the attached is less 'messy', but at leaset the monthly formulas are shorter.

Also Note: I believe your formula on the rate sheet in cell C9 is missing the data point from the entry sheet in cell O6
