Index/Match ignoring Blanks/Zeros for each instance

    Greetings Programs!

I've spent some time playing with some formulas without much success, but I'm sure someone here will think of something I haven't.

I want to be able to show each instance that there is an amount in column K and/or column M on the Report tab on the Upload tab in column A (not a sum though). The criteria is the Home Department (column B on Report and column C on Upload). I would also like to show the File Number associated if an amount is not 0/Blank in column E on the Upload tab.

The actual Report may have different dates so it might be helpful to have a second condition be the date column (column A on Report and column D on Upload).

Thank you very much for any assistance.
