Summary of a particular Code from Data table - outlined per entry

Hi All

Below is a Data set of information with account does & amounts related to each code:

I am looking to do a summary for A/C 3131 for more information to be added manually for each line. Ideally I would like a table that lists each entry on a separate line for A/C 3131 using a formula to output the following:

I have a feeling an using IF would be the way but unsure of the approach to get in as a neat data set with no line breaks (just trying to avoid manual manipulation of data output).

Any direction/assistance would be greatly appreciated :-)

I have attached the references listed above in the attached Excel Book. The top table is the data and the 2nd table is out I am looking to output for A/C 3131.
