How to Keep only the first 4 rows to each unique identifier

I need to keep only the first 4 rows associated with a unique identifier. I would like to do this with a formula or function, and avoid Macros as I do not know them at this time.

I have excel sheets that contain files of providers with each provider having anywhere from 1 to 20 rows. I want to only keep the first 4 (sometimes 5) rows listed. Each provider has a unique ID number repeated on each of these rows.
