I have a form where users will enter a date in MMM-DD-YYYY format (APR-01-2015) and I need it to populate in another cell in a different format (MM/DD/YYYY).
Can one of you experts please help? I attach an example:

=SUBSTITUTE(C7,"-", ", ")+0
Format the cell with the formula as mm/dd/yyyy
Yes, that does it, can you please add it with my formula that has the if statement pls? so basically if C7 = blank then blank the cell (as per the attachement)

Try it with C7 blank...
The formula will be blank as well.
here you go:
=IF(C7="","",SUBSTITUTE(C7,"-",", ")+0)
