Retrieval of values from columns with gaps between month columns

HI,

I would like to do the following.

I would like to retrieve for each category A, C and D (categories appears several times) in the target tab "fee" the monthly values in separate columns

I attach an example in an excelfile as well as a word document showing the lookup problem.
