Formula to Calculate amount based on the start and end date

    Hi Friends

I want to enter the formula in the attached file in the yellow section. The formula should look the date and calculate the amount based on the month in row 3. It should also increase on each anniversary.

Can someone please help to work out the formula?

    Thanks a lot in advance.

    Regards
    Arpan
