Matching Sample Column with Actual Name Column and Show Result in Next Column

    Hi all,
I am trying to convert (matching) the encoded place names of survey (Column B) which is about 690 names with the actual place names (Column C) and show results in Column D for the encoded survey names (about 690 names). I can't figure out the best formula considering that there are duplicates and similar names in the columns. I have attached the worksheet that I am working (hoping that the attachment got through). Thank you very much for your help.
