Extract list without duplicates according 3 criteria

Hi there,

Do we have a formula to extract a list without duplicates accroding to 3 criteria.

I have attached an example of what I want:

sheet2, I would like the list of all the tests that the company 23569 does in Sydney for the type '10'. I want a list without duplicate.

A pivot table do that perfeclty but I need to create an automated report so I just want to have to update my database.
