Extract numbers from 4 sheets if date available

Hello Friends

Please refer the attached excel 2010 file.

I want to extract the values from 4 sheets (Sheet1 to Sheet4) in Column H if corresponding Column J have date.

I have plotted the expected result by formula in Sheet 5 light yellow and light green cells.
