Formula Needed to populate info from one sheet to another based on cell entry, WS attached

This one is a little tricky.

I need a formula that will populate the info from certain cells on a row in one sheet into certain cells on another sheet based on a number being entered into a cell.

On sheet “LG”, when the number 1 is entered into H6:H1005, The info existing under titles: Date, #, Type, First, Last, and I.D. in that row will populate into sheet “LEADS” under the same titles.

The LEADS sheet will populate the info in the next row and so on as it is entered on sheet LG based on the number 1 being entered in H6:H1005.

An option is to insert your data via a Pivot Table.

Attached is a version of your file with a Pivot Table displaying the data as requested.
You will need to refresh the Pivot Table as necessary or get a macro that will refresh it automatically.

It works on the sheet you sent but when I try to follow the steps on my sheet i cannot get it to work all the way down to 1005.
