Matching row criteria with header

Hi Everyone,

I'm trying to lookup for instance item "2222" with size "12", this item showed a date of 2/14/2017. Is there a formula i can use pull this data.

and on the same token item code "4444" with size "64" should be pulling 2/17/17 date.

You have posted a very poor sample. All instances of 2222 are size twelve and all corresponding dates are 2/14/2017
