Index match formula needed !

Hi Everyone,

I need a formula to plug into C15, C18, and C21 that I could drag across.

In C15, It needs to pull the data from row 4 with reference to the cell of A14.
In C18 it needs to pull the data from row 5 with reference to the cell of A17.

And so on and so forth.

Can someone please help me with this

Yes - my account stopped working so i had to create a new one. It was weird
