Advance filter between 2 dates

Trying to work through an advanced Filter.
How do I complete the filter between 2 dates and also filter if the name is there twice. I have an appointment booking database and want to create an alert if the person has been booked X amount of times in a week(lets say 3)

Headings are:
Date of Appointment & Name

Not sure how to complete both of these functions in one Advanced Filter

So I would like for the filter to only show 1-the bookings between the two dates(dates are pulled from another worksheet) and 2- if there are any people with more then 2 bookings during that time period.


The third part would be also get some help to create an alert box if some sort to notify the user.

I am hoping to get this done without vba but if it is a must I will take a crack at it.
