Need help on additional search box.

Hi,

Recently i been testing with new auto form and so far everything is in order. Refer to file attached, J3 cell is to search "Service Chit No".
How can i add or create another same search box like J3 but i want to search "Reference" on cell H3

Can anyone assist? Really appreciate if anyone out there can assist. This is a new template for my company data. Its very important sheet.

Btw i'm excel dummy. This attachment i download it some where and i modified it to my company usage.
