Modfication of Existing Code for additional additional cells

I added the extra columns (C1:L1) in `outputs' sheet and try to modify the existing code and getting error.

My requirement is as follows :
Please find the 2 sheets `Inputs' and `Outputs' in attached file
I had a very huge excel file and here made a simple excel file for example.
I need to input the various dates in (`inputs' sheet cell B1 from `inputs' sheet cell B4:B103) one by one.
And I need to take the ouputs (from `outputs' sheet cell C1 and L1 and copy paste to `outputs' sheet cell table C4:L103) one by one.

Kindly help me to correct the code.

Hello experts,

I need a quick support from this group.
I am in need to create a simple Excel Tool that should have the following:
1) Questions with Answer Options
2) Set of Questions would be under some categories which will have some weightages
3) Each Answer Options will have some specific scores
4) After completion of filling the questionnaire, the result should reflect in an overall score (may be out of 100)

Can someone help me on this? I think a re-usable macro enabled sheet (in the similar line) should work with some editing/customization that I will do.
