Lookup Work Only in Box 1, Not Fully Work

hi all..

i have a problem a data lookup, i have 4 box contains name that i want lookup
i have use array funct: =IFERROR(INDEX($L:$L;SMALL(IF($K$3:$K$568=$F$2;ROW($K$3:$K$13));ROWS(G$4:G4)));"")
but only Box 1 as work, for the other box can not show text (not working)..
